Q. Where Can I Find Form 1023-EZ?

A. You can view Form 1023-EZ on the pay.gov website.  Enter 1023-EZ in the search box, click through, and when the page listing the steps to complete the form appears, scroll down to "Preview Form" near the bottom of the page of the left side.     You might want to print a copy to use as a rough draft.   The same page has a link to the 1023-EZ instructions.

Like the long-form 1023, and now Form 8940 (Miscellaneous Changes), Form 1023-EZ can only be filed on-line.  The first step in doing this is to register at Pay.gov.​
